WHAT WE DO...

Care4All Community Care, Inc. is a licensed Private Home Care agency (PHC) providing quality in-home Personal Assistance through Certified Nursing Assistants (CNA). These services make it possible for individuals who are elderly and/or functionally impaired to remain in their homes and/or communities. We are an Elderly and Disabled Waiver Program (EDWP) provider. For eligible individuals EDWP offers community-based services as an alternative to nursing facility placement.

Medicaid eligible or potentially Medicaid eligible individuals are required for EDWP.  SOURCE and CCSP are designed to assist those individuals who are elderly and/or functionally impaired remain in their homes and receive services as an alternative to nursing homes.
